**May 2024 Zion HealthShare Member Guidelines Updates**

To keep membership contributions low for all members, we implement a waiting period for sharing of medical conditions that existed prior to a member’s membership start date. We call such medical conditions: pre-membership medical conditions. This section defines pre-membership medical conditions and outlines related sharing limitations.

Annual provider visits and other preventive services are available to you without the need to meet your IUA.

One annual Provider visit per year up to $175 shared.

One preventive, annual mammogram at no additional cost beginning at age 40. Mammograms are shareable after 6 months of continuous membership.

Colonoscopies are shareable every 10 years starting at age 45. Colonoscopies are shareable after 6 months of continuous membership.

Youth Immunizations (up to 18 years old) are sharable after 6 months of continuous membership or upon birth if born through an eligible Maternity Sharing Request.

Comprehensive and Catastrophic members that become pregnant after their first day of enrollment will be covered immediate for sharing.

Abortions and fertility treatments are not considered to be a sharable need.

Sharing requests should be submitted through the Member Portal, as soon as possible. Most nonemergency sharing requests, such as surgical procedures, should be submitted prior to the date of service.

In order to be eligible for sharing, sharing requests and each itemized bill and statement must be submitted within six months of the date of service. The IUA must also be paid within six months to be considered for sharing. When submitting a sharing request, members should submit any documentation received from their provider. Zion HealthShare may request additional documents that may be required throughout the sharing request process. Document types can include, but are not limited to, receipts, proofs of payment, itemized bills and statements.
Original, itemized bills and statements should be submitted promptly to Zion HealthShare along with the sharing request form for us to process the sharing request as soon as possible.

The initial unshareable amount, or IUA, is the amount a member will pay per medical need before the Zion HealthShare community shares in medical expenses. Zion HealthShare has three IUA options: $1,000, $2,500, and $5,000. The lower your IUA, the higher your monthly contribution will be.

Medical expenses are only shareable after members have met their IUA. Members should provide documentation to Zion HealthShare of all payments that may contribute toward the member’s IUA.

The IUA must be paid within six months of the date of service or bills may become ineligible for sharing. Consideration will be given for situations where the cost of treatment has not exceeded the IUA after six months for certain sharing requests.
